Job Summary: The Licensed Practical Nurse (LPN) is responsible for providing direct nursing care, monitoring the health care of individuals, and assisting individuals with accessing appropriate health care within the LPN scope of practice. The LPN is also responsible for monitoring the medication management. Clinical assistance is under the direction of the RN Nurse Manager.

Licensed Practical/Vocational Nurse (LPN/LVN)

· Brockton Home Health Care is quickly expanding and we are hiring Homecare Licensed Practical Nurses (LPN/LVN) - full-time, part-time, and per diem effective immediately. We care for patients of all ages and backgrounds who may require daily visits in their homes in the surrounding areas:

• Area: Holyoke, Springfield, Chicopee, Westfield, West Springfield

Licensed Practical Nurses (LPN/LVN) Responsibilities

• Assess patient's condition during every visit and document your observations
• Administer medication as prescribed by the patient’s Physicians perform wound care, cathetar change, disease management
• Coordinate with Physicians and anyone else involved with the patient’s care plan
• Accurately document observations, interventions, and evaluations pertaining to client care management
